NEWS UPDATE March 10, 2009 If you would like to appeal the claim decision reached by the General Motors Customer Communications Centre, you must submit an appeal form to the Claims Administrator. The appeal form is named "Request for Review of Rejected Claim Form" (Annex I) and can be found under the Claim Documents tab. The Claims Administrator's mailing address is on the bottom of the form or can be found below. If you wish to appeal the amount awarded for your claim you will need to use the Request for Review of Rejected Claim Form, as there is no other appeal form available under this settlement. October 31, 2008 The Settlement Agreement has been approved by the Courts. CLAIM FORMS can be printed from the website (under Claim Documents) or obtained by contacting the General Motors Customer Communications Centre, toll free 1-800-263-3777 (English) or 1-800-263-7854 (French).
